Download and complete your new patient forms before you arrive to save time. Bring your completed forms, photo ID, and insurance card to your appointment.
Your Comprehensive Exam
Your first visit includes a thorough oral exam, digital X-rays, a periodontal (gum) evaluation, and an oral cancer screening — a complete picture of your dental health.
Your Personalized Treatment Plan
Your doctor will review your exam findings with you, answer all your questions, and create a customized treatment plan that fits your goals and budget.
What to Bring
- Completed new patient forms (or arrive 15 minutes early to fill them out)
- Photo ID (driver's license, state ID, or passport)
- Dental insurance card(s)
- List of current medications and dosages

How early should I arrive for my first appointment?
If you haven't completed your forms in advance, please arrive 15 minutes early. If your forms are done, arriving 5 minutes before your scheduled time is fine.
How long will my first appointment take?
New patient comprehensive exams typically take 60–90 minutes. This gives us enough time to thoroughly evaluate your oral health and have a meaningful conversation about your goals.
Do you see children?
Yes! Italian Smiles is a family dental practice. We welcome patients of all ages and love helping families maintain healthy smiles together.
What if I haven't been to the dentist in a long time?
